You can use the links below to find a treatment provider near you. Make sure you are using a UTAH COURT APPROVED PROVIDER or you will not receive credit for the treatment.
How Can I Submit an Assessment?
There are two possible ways you can submit an assessment to the court:
- You can either bring the assessment to the court by hand, OR
- Your treatment provider can submit the assessment directly to the court before the hearing date.
The judge reviews the assessment BEFORE your hearing, so providing it at least 24 hours before the hearing is preferred and sometimes ordered by the court.
NOTE: If your treatment provider tells you they will submit the assessment directly to the court, you still have the responsibility to contact the court and double-check that your provider submitted the assessment before your hearing date. You may contact the court or submit an assessment at any time during regular business hours, using the information below:
